Support Coordination
We provide assistance to implement your NDIS plan using Level 2 – Coordination of Supports within your NDIS approved plan. We aim to collaborate with you so you can utilise your plan to reach your goals and build your capacity.
Currently Whitehorse Disability Supports can assist participants with NDIS plans that are self-managed.
We can assist with:
- Preparation support in readiness for NDIS meetings and reviews
- Identify services that meet your individual needs
- Provide information on appropriate funding supports
- Connect with service providers and supports
- Liaise with NDIS and related service providers
- Link you to relevant community and employment supports
- Assist with organisation of supports, obtain relevant quotes and ensure service agreements are appropriate
- Implementation of your NDIS plan, understand your funded supports, budget, categories and how to maximise your funding
